perspective from discharge
planning and executing to true
daily utilization management
review of the
patients. The goal of
care management is to use
coordinated healthcare services
to promote quality care, patient
satisfaction, and cost-effective
outcomes. Care management is a
collaborative process that
involves assessing, planning,
implementing, coordinating,
monitoring, and evaluating
options and services needed to
meet a patient's health

case manager, the patient, the
patient's family, and the
patient's healthcare providers
work as a team to develop a care
plan, and patient-specific goals
that will enable the patient to
return to the highest possible
level of wellness. Our energy and
interest is focused on staffing
the ideal registered nurses and
social workers as talented case
managers. This process becomes a
beneficial partnership for those
involved in care management on
many levels. Our case
managers are experts in all
aspects of insurance
coverage. Each one
communicates with the insurance
companies to review your plan of
care and hospital coverage and to
clarify exactly what your health
care plan covers. Case
managers have a responsibility to
advance the patient's best
interests within the limits of
their insurance program's
policies. Case managers
facilitate the effort and help
identify appropriate providers
and facilities throughout the
continuum of services. Your case
manager will then be able to help
you plan for discharge needs
accordingly. Finally, the
case manager facilitates and
communicates treatment and
discharge planning with you, your
family and all members of the
health care team. Sunflower
balances the needs of our staff
and clients so we can provide
maximum care and coverage. Our
case managers are usually
assigned to specific
medical-surgical areas of the
hospital and help cover the
intensive-coronary care unit,
neurology unit, telemetry unit
and in some instances the
emergency department. We only
service a select group of clients
which allows us to maintain
staffing requests throughout the
Tucson community. Please ask
about our new discount plan for
block scheduling, follow-up
visits and monthly services. At
certain times, our staff will be
available to fill in for
emergency
cancellations. We encourage
the use of the Internet for
scheduling and cancellations. All
staffing schedules
and services
are usually discussed by phone
and then confirmed by e-mail.

understanding what to do for any
reason. Registered
nurses and social workers with
occupational and / or injured
workers' compensation experience
are in great demand. If it makes
sense to be considered for an
