Sunflower Health Services provides quality care
management, per diem staffing, at competitive
rates. We seek to cultivate long-term relationships
with our clients in acute care hospitals, skilled
nursing homes, assisted living facility and private
home settings.
We only service a select group of clients which
allows us to maintain staffing requests throughout
the Tucson community. Our energy and interest is
focused on staffing the ideal registered nurses and
social workers. The process becomes a beneficial
partnership for all involved.
Sunflower balances the needs of our staff and
clients so we can provide maximum care and
coverage. Employees are assigned to specific
medical-surgical areas of the hospital and help
cover the intensive-coronary care unit, emergency
department, and admitting department.
Care management is
the backbone of any hospital -- it drives patient
care as well as the performance of the physician
team. At Sunflower, we solidify this concept
hospital-wide by changing the care management
perspective from discharge planning to true daily
care management of the patient.
Our staff actively manages patient care from
admission through the entire continuum of care. It
should be a collaborative process that assesses,
plans, implements, coordinates, monitors and
evaluates the options and services required to meet
a patient's health needs, using communication and
available resources to
promote quality, cost-effective outcomes.
Case Managers frequently consult with
physicians, nursing staff, administration and
ancillary staff to ensure that proper care is
provided at the right time and at the right level.
All inpatients and observation patients are
reviewed at the time of admission to ensure that
they meet inpatient admission criteria. Also
assessed at the time of admission are potential
barriers to discharge.
Sunflower ensures the facilitation of core
measure competency to
maximize the quality of care and thus the acute
care hospital's reimbursement. We also look at the
continuum of care while patients are in the
hospital and their post-acute care needs.
At certain times, our staff will be available to
fill in for emergency cancellations. We encourage
the use of the Internet for scheduling
and cancellations. All staffing schedules are
discussed by phone and then confirmed by e-mail. We
